The SIAR and SDAR registers are available twice, one time as SPRs
780 / 781 (unprivileged, but read-only), and one time as the SPRs
796 / 797 (privileged, but read and write). The Linux kernel code
currently uses the unprivileged SPRs - while this is OK for reading,
writing to that register of course does not work.
Since the KVM code tries to write to this register, too (see the mtspr
in book3s_hv_rmhandlers.S), the contents of this register sometimes get
lost for the guests, e.g. during migration of a VM.
To fix this issue, simply switch to the privileged SPR numbers instead.

Some of the interrupt vectors on 64-bit POWER server processors are only
32 bytes long (8 instructions), which is not enough for the full
first-level interrupt handler. For these we need to branch to an
out-of-line (OOL) handler. But when we are running a relocatable kernel,
interrupt vectors till __end_interrupts marker are copied down to real
address 0x100. So, branching to labels (ie. OOL handlers) outside this
section must be handled differently (see LOAD_HANDLER()), considering
relocatable kernel, which would need at least 4 instructions.
However, branching from interrupt vector means that we corrupt the
CFAR (come-from address register) on POWER7 and later processors as
mentioned in commit 1707dd16. So, EXCEPTION_PROLOG_0 (6 instructions)
that contains the part up to the point where the CFAR is saved in the
PACA should be part of the short interrupt vectors before we branch out
to OOL handlers.
But as mentioned already, there are interrupt vectors on 64-bit POWER
server processors that are only 32 bytes long (like vectors 0x4f00,
0x4f20, etc.), which cannot accomodate the above two cases at the same
time owing to space constraint. Currently, in these interrupt vectors,
we simply branch out to OOL handlers, without using LOAD_HANDLER(),
which leaves us vulnerable when running a relocatable kernel (eg. kdump
case). While this has been the case for sometime now and kdump is used
widely, we were fortunate not to see any problems so far, for three
reasons:
1. In almost all cases, production kernel (relocatable) is used for
kdump as well, which would mean that crashed kernel's OOL handler
would be at the same place where we end up branching to, from short
interrupt vector of kdump kernel.
2. Also, OOL handler was unlikely the reason for crash in almost all
the kdump scenarios, which meant we had a sane OOL handler from
crashed kernel that we branched to.
3. On most 64-bit POWER server processors, page size is large enough
that marking interrupt vector code as executable (see commit
429d2e83) leads to marking OOL handler code from crashed kernel,
that sits right below interrupt vector code from kdump kernel, as
executable as well.
Let us fix this by moving the __end_interrupts marker down past OOL
handlers to make sure that we also copy OOL handlers to real address
0x100 when running a relocatable kernel.
This fix has been tested successfully in kdump scenario, on an LPAR with
4K page size by using different default/production kernel and kdump
kernel.
Also tested by manually corrupting the OOL handlers in the first kernel
and then kdump'ing, and then causing the OOL handlers to fire - mpe.

The REAL_LE feature entry in the ibm_pa_feature struct is missing an MMU
feature value, meaning all the remaining elements initialise the wrong
values.
This means instead of checking for byte 5, bit 0, we check for byte 0,
bit 0, and then we incorrectly set the CPU feature bit as well as MMU
feature bit 1 and CPU user feature bits 0 and 2 (5).
Checking byte 0 bit 0 (IBM numbering), means we're looking at the
"Memory Management Unit (MMU)" feature - ie. does the CPU have an MMU.
In practice that bit is set on all platforms which have the property.
This means we set CPU_FTR_REAL_LE always. In practice that seems not to
matter because all the modern cpus which have this property also
implement REAL_LE, and we've never needed to disable it.
We're also incorrectly setting MMU feature bit 1, which is:

Luckily the only place that looks for MMU_FTR_TYPE_8xx is in Book3E
code, which can't run on the same cpus as scan_features(). So this also
doesn't matter in practice.
Finally in the CPU user feature mask, we're setting bits 0 and 2. Bit 2
is not currently used, and bit 0 is:
#define PPC_FEATURE_PPC_LE 0x00000001
Which says the CPU supports the old style "PPC Little Endian" mode.
Again this should be harmless in practice as no 64-bit CPUs implement
that mode.
Fix the code by adding the missing initialisation of the MMU feature.
Also add a comment marking CPU user feature bit 2 (0x4) as reserved. It
would be unsafe to start using it as old kernels incorrectly set it.

Since binutils 2.26 BFD is doing suffix merging on STRTAB sections. But
dedotify modifies the symbol names in place, which can also modify
unrelated symbols with a name that matches a suffix of a dotted name. To
remove the leading dot of a symbol name we can just increment the pointer
into the STRTAB section instead.
Backport to all stables to avoid breakage when people update their
binutils - mpe.

According to memory-barriers.txt, xchg*, cmpxchg* and their atomic_
versions all need to be fully ordered, however they are now just
RELEASE+ACQUIRE, which are not fully ordered.
So also replace PPC_RELEASE_BARRIER and PPC_ACQUIRE_BARRIER with
PPC_ATOMIC_ENTRY_BARRIER and PPC_ATOMIC_EXIT_BARRIER in
__{cmp,}xchg_{u32,u64} respectively to guarantee fully ordered semantics
of atomic{,64}_{cmp,}xchg() and {cmp,}xchg(), as a complement of commit
b97021f855 ("powerpc: Fix atomic_xxx_return barrier semantics")
This patch depends on patch "powerpc: Make value-returning atomics fully
ordered" for PPC_ATOMIC_ENTRY_BARRIER definition.

According to memory-barriers.txt:
> Any atomic operation that modifies some state in memory and returns
> information about the state (old or new) implies an SMP-conditional

Which mean these operations should be fully ordered. However on PPC,
PPC_ATOMIC_ENTRY_BARRIER is the barrier before the actual operation,
which is currently "lwsync" if SMP=y. The leading "lwsync" can not
guarantee fully ordered atomics, according to Paul Mckenney:

To fix this, we define PPC_ATOMIC_ENTRY_BARRIER as "sync" to guarantee
the fully-ordered semantics.
This also makes futex atomics fully ordered, which can avoid possible
memory ordering problems if userspace code relies on futex system call
for fully ordered semantics.

Currently we allow both the MSR T and S bits to be set by userspace on
a signal return. Unfortunately this is a reserved configuration and
will cause a TM Bad Thing exception if attempted (via rfid).
This patch checks for this case in both the 32 and 64 bit signals
code. If both T and S are set, we mark the context as invalid.
Found using a syscall fuzzer.

Currently it is possible for userspace (e.g. QEMU) to set a value
for the MSR for a guest VCPU which has both of the TS bits set,
which is an illegal combination. The result of this is that when
we execute a hrfid (hypervisor return from interrupt doubleword)
instruction to enter the guest, the CPU will take a TM Bad Thing
type of program interrupt (vector 0x700).
Now, if PR KVM is configured in the kernel along with HV KVM, we
actually handle this without crashing the host or giving hypervisor
privilege to the guest; instead what happens is that we deliver a
program interrupt to the guest, with SRR0 reflecting the address
of the hrfid instruction and SRR1 containing the MSR value at that
point. If PR KVM is not configured in the kernel, then we try to
run the host's program interrupt handler with the MMU set to the
guest context, which almost certainly causes a host crash.
This closes the hole by making kvmppc_set_msr_hv() check for the
illegal combination and force the TS field to a safe value (00,
meaning non-transactional).

This fixes a race which can result in the same virtual IRQ number
being assigned to two different MSI interrupts. The most visible
consequence of that is usually a warning and stack trace from the
sysfs code about an attempt to create a duplicate entry in sysfs.
The race happens when one CPU (say CPU 0) is disposing of an MSI
while another CPU (say CPU 1) is setting up an MSI. CPU 0 calls
(for example) pnv_teardown_msi_irqs(), which calls
msi_bitmap_free_hwirqs() to indicate that the MSI (i.e. its
hardware IRQ number) is no longer in use. Then, before CPU 0 gets
to calling irq_dispose_mapping() to free up the virtal IRQ number,
CPU 1 comes in and calls msi_bitmap_alloc_hwirqs() to allocate an
MSI, and gets the same hardware IRQ number that CPU 0 just freed.
CPU 1 then calls irq_create_mapping() to get a virtual IRQ number,
which sees that there is currently a mapping for that hardware IRQ
number and returns the corresponding virtual IRQ number (which is
the same virtual IRQ number that CPU 0 was using). CPU 0 then
calls irq_dispose_mapping() and frees that virtual IRQ number.
Now, if another CPU comes along and calls irq_create_mapping(), it
is likely to get the virtual IRQ number that was just freed,
resulting in the same virtual IRQ number apparently being used for
two different hardware interrupts.
To fix this race, we just move the call to msi_bitmap_free_hwirqs()
to after the call to irq_dispose_mapping(). Since virq_to_hw()
doesn't work for the virtual IRQ number after irq_dispose_mapping()
has been called, we need to call it before irq_dispose_mapping() and
remember the result for the msi_bitmap_free_hwirqs() call.
The pattern of calling msi_bitmap_free_hwirqs() before
irq_dispose_mapping() appears in 5 places under arch/powerpc, and
appears to have originated in commit 05af7bd2d7 ("[POWERPC] MPIC
U3/U4 MSI backend") from 2007.

The EPOW interrupt handler uses rtas_get_sensor(), which in turn
uses rtas_busy_delay() to wait for RTAS becoming ready in case it
is necessary. But rtas_busy_delay() is annotated with might_sleep()
and thus may not be used by interrupts handlers like the EPOW handler!
This leads to the following BUG when CONFIG_DEBUG_ATOMIC_SLEEP is

Fix this issue by introducing a new rtas_get_sensor_fast() function
that does not use rtas_busy_delay() - and thus can only be used for
sensors that do not cause a BUSY condition - known as "fast" sensors.
The EPOW sensor is defined to be "fast" in sPAPR - mpe.

The powerpc kernel can be built to have either a 4K PAGE_SIZE or a 64K
PAGE_SIZE.
However when built with a 4K PAGE_SIZE there is an additional config
option which can be enabled, PPC_HAS_HASH_64K, which means the kernel
also knows how to hash a 64K page even though the base PAGE_SIZE is 4K.
This is used in one obscure configuration, to support 64K pages for SPU
local store on the Cell processor when the rest of the kernel is using
4K pages.
In this configuration, pte_pagesize_index() is defined to just pass
through its arguments to get_slice_psize(). However pte_pagesize_index()
is called for both user and kernel addresses, whereas get_slice_psize()
only knows how to handle user addresses.
This has been broken forever, however until recently it happened to
work. That was because in get_slice_psize() the large kernel address
would cause the right shift of the slice mask to return zero.
However in commit 7aa0727f33 ("powerpc/mm: Increase the slice range to
64TB"), the get_slice_psize() code was changed so that instead of a
right shift we do an array lookup based on the address. When passed a
kernel address this means we index way off the end of the slice array
and return random junk.
That is only fatal if we happen to hit something non-zero, but when we
do return a non-zero value we confuse the MMU code and eventually cause
a check stop.
This fix is ugly, but simple. When we're called for a kernel address we
return 4K, which is always correct in this configuration, otherwise we
use the slice mask.

This function can leak kernel stack data when the user siginfo_t has a
positive si_code value. The top 16 bits of si_code descibe which fields
in the siginfo_t union are active, but they are treated inconsistently
between copy_siginfo_from_user32, copy_siginfo_to_user32 and
copy_siginfo_to_user.
copy_siginfo_from_user32 is called from rt_sigqueueinfo and
rt_tgsigqueueinfo in which the user has full control overthe top 16 bits
of si_code.
This fixes the following information leaks:
x86: 8 bytes leaked when sending a signal from a 32-bit process to
itself. This leak grows to 16 bytes if the process uses x32.
(si_code = __SI_CHLD)
x86: 100 bytes leaked when sending a signal from a 32-bit process to
a 64-bit process. (si_code = -1)
sparc: 4 bytes leaked when sending a signal from a 32-bit process to a
64-bit process. (si_code = any)
parsic and s390 have similar bugs, but they are not vulnerable because
rt_[tg]sigqueueinfo have checks that prevent sending a positive si_code
to a different process. These bugs are also fixed for consistency.

When we take a PMU exception or a software event we call
perf_read_regs(). This overloads regs->result with a boolean that
describes if we should use the sampled instruction address register
(SIAR) or the regs.
If the exception is in kernel, we start with the kernel regs and
backtrace through the kernel stack. At this point we switch to the
userspace regs and backtrace the user stack with perf_callchain_user().
Unfortunately these regs have not got the perf_read_regs() treatment,
so regs->result could be anything. If it is non zero,
perf_instruction_pointer() decides to use the SIAR, and we get issues
like this:

Notice the bogus _raw_spin_irqsave when we transition from kernel
(system_call) to userspace (0x7e714). We inserted what was in the SIAR.
Add a check in regs_use_siar() to check that the regs in question
are from a PMU exception. With this fix the backtrace makes sense:

Recent toolchains force the TOC to be 256 byte aligned. We need
to enforce this alignment in our linker script, otherwise pointers
to our TOC variables (__toc_start, __prom_init_toc_start) could
be incorrect.
If they are bad, we die a few hundred instructions into boot.

The core VM already knows about VM_FAULT_SIGBUS, but cannot return a
"you should SIGSEGV" error, because the SIGSEGV case was generally
handled by the caller - usually the architecture fault handler.
That results in lots of duplication - all the architecture fault
handlers end up doing very similar "look up vma, check permissions, do
retries etc" - but it generally works. However, there are cases where
the VM actually wants to SIGSEGV, and applications _expect_ SIGSEGV.
In particular, when accessing the stack guard page, libsigsegv expects a
SIGSEGV. And it usually got one, because the stack growth is handled by
that duplicated architecture fault handler.
However, when the generic VM layer started propagating the error return
from the stack expansion in commit fee7e49d4514 ("mm: propagate error
from stack expansion even for guard page"), that now exposed the
existing VM_FAULT_SIGBUS result to user space. And user space really
expected SIGSEGV, not SIGBUS.
To fix that case, we need to add a VM_FAULT_SIGSEGV, and teach all those
duplicate architecture fault handlers about it. They all already have
the code to handle SIGSEGV, so it's about just tying that new return
value to the existing code, but it's all a bit annoying.
This is the mindless minimal patch to do this. A more extensive patch
would be to try to gather up the mostly shared fault handling logic into
one generic helper routine, and long-term we really should do that
cleanup.
Just from this patch, you can generally see that most architectures just
copied (directly or indirectly) the old x86 way of doing things, but in
the meantime that original x86 model has been improved to hold the VM
semaphore for shorter times etc and to handle VM_FAULT_RETRY and other
"newer" things, so it would be a good idea to bring all those
improvements to the generic case and teach other architectures about
them too.

Commit 746c9e9f92dd "of/base: Fix PowerPC address parsing hack" limited
the applicability of the workaround whereby a missing ranges is treated
as an empty ranges. This workaround was hiding a bug in the etsec2
device tree nodes, which have children with reg, but did not have
ranges.

Anton has a busy ppc64le KVM box where guests sometimes hit the infamous
"kernel BUG at kernel/smpboot.c:134!" issue during boot:
BUG_ON(td->cpu != smp_processor_id());
Basically a per CPU hotplug thread scheduled on the wrong CPU. The oops
output confirms it:

Comm: watchdog/130
The problem is that we aren't ensuring the CPU active bit is set for the
secondary before allowing the master to continue on. The master unparks
the secondary CPU's kthreads and the scheduler looks for a CPU to run
on. It calls select_task_rq() and realises the suggested CPU is not in
the cpus_allowed mask. It then ends up in select_fallback_rq(), and
since the active bit isnt't set we choose some other CPU to run on.
This seems to have been introduced by 6acbfb96976f "sched: Fix hotplug
vs. set_cpus_allowed_ptr()", which changed from setting active before
online to setting active after online. However that was in turn fixing a
bug where other code assumed an active CPU was also online, so we can't
just revert that fix.
The simplest fix is just to spin waiting for both active & online to be
set. We already have a barrier prior to set_cpu_online() (which also
sets active), to ensure all other setup is completed before online &
active are set.

The commit 3b8a3c010969 ("powerpc/pseries: Fix endiannes issue in RTAS
call from xmon") was fixing an endianness issue in the call made from
xmon to RTAS.
However, as Michael Ellerman noticed, this fix was not complete, the
token value was not byte swapped. This lead to call an unexpected and
most of the time unexisting RTAS function, which is silently ignored by
RTAS.
This fix addresses this hole.

Commit 5d26a105b5a7 ("crypto: prefix module autoloading with "crypto-"")
changed the automatic module loading when requesting crypto algorithms
to prefix all module requests with "crypto-". This requires all crypto
modules to have a crypto specific module alias even if their file name
would otherwise match the requested crypto algorithm.
Even though commit 5d26a105b5a7 added those aliases for a vast amount of
modules, it was missing a few. Add the required MODULE_ALIAS_CRYPTO
annotations to those files to make them get loaded automatically, again.
This fixes, e.g., requesting 'ecb(blowfish-generic)', which used to work
with kernels v3.18 and below.
Also change MODULE_ALIAS() lines to MODULE_ALIAS_CRYPTO(). The former
won't work for crypto modules any more.

The root cause is that xmon is calling RTAS to turn off the surveillance

Unlike global OOM handling, memory cgroup code will invoke the OOM killer
in any OOM situation because it has no way of telling faults occuring in
kernel context - which could be handled more gracefully - from
user-triggered faults.
Pass a flag that identifies faults originating in user space from the
architecture-specific fault handlers to generic code so that memcg OOM
handling can be improved.

ABIv2 kernels are failing to backtrace through the kernel. An example:

The problem is in valid_next_sp() where we check that the new stack
pointer is at least STACK_FRAME_OVERHEAD below the previous one.
ABIv1 has a minimum stack frame size of 112 bytes consisting of 48 bytes
and 64 bytes of parameter save area. ABIv2 changes that to 32 bytes
with no paramter save area.
STACK_FRAME_OVERHEAD is in theory the minimum stack frame size,
but we over 240 uses of it, some of which assume that it includes
space for the parameter area.
We need to work through all our stack defines and rationalise them
but let's fix perf now by creating STACK_FRAME_MIN_SIZE and using
in valid_next_sp(). This fixes the issue:

On ppc64 we support 4K hash pte with 64K page size. That requires
us to track the hash pte slot information on a per 4k basis. We do that
by storing the slot details in the second half of pte page. The pte bit
_PAGE_COMBO is used to indicate whether the second half need to be
looked while building real_pte. We need to use read memory barrier while
doing that so that load of hidx is not reordered w.r.t _PAGE_COMBO
check. On the store side we already do a lwsync in __hash_page_4K

In addition to bringing in upstream commits, this merge also makes minor
changes to mainitain compatibility with upstream:
The definition of list_next_entry in qcrypto.c and ipa_dp.c has been
removed, as upstream has moved the definition to list.h. The implementation
of list_next_entry was identical between the two.
irq.c, for both arm and arm64 architecture, has had its calls to
__irq_set_affinity_locked updated to reflect changes to the API upstream.
Finally, as we have removed the sleep_length member variable of the
tick_sched struct, all changes made by upstream commit ec804bd do not
apply to our tree and have been removed from this merge. Only
kernel/time/tick-sched.c is impacted.

The optimistic spin code assumes regular stores and cmpxchg() play nice;
this is found to not be true for at least: parisc, sparc32, tile32,
metag-lock1, arc-!llsc and hexagon.
There is further wreckage, but this in particular seemed easy to
trigger, so blacklist this.
Opt in for known good archs.

Move the /memreserve/ processing and dtb memory reservations into
early_init_fdt_scan_reserved_mem. This converts arm, arm64, and powerpc
as they are the only users of early_init_fdt_scan_reserved_mem.
memblock_reserve is safe to call on the same region twice, so the
reservation check for the dtb in powerpc 32-bit reservations is safe to
remove.

Both powerpc and microblaze have the same FDT blob in debugfs feature.
Move this to common location and remove the powerpc and microblaze
implementations. This feature could become more useful when FDT
overlay support is added.
This changes the path of the blob from "$arch/flat-device-tree" to
"device-tree/flat-device-tree".

On POWER8 when switching to a KVM guest we set bits in MMCR2 to freeze
the PMU counters. Aside from on boot they are then never reset,
resulting in stuck perf counters for any user in the guest or host.
We now set MMCR2 to 0 whenever enabling the PMU, which provides a sane
state for perf to use the PMU counters under either the guest or the
host.
This was manifesting as a bug with ppc64_cpu --frequency:

The command uses a perf counter to measure CPU cycles over a fixed
amount of time, in order to approximate the frequency of the machine.
The counters were returning zero once a guest was started, regardless of
weather it was still running or had been shut down.
By dumping the value of MMCR2, it was observed that once a guest is
running MMCR2 is set to 1s - which stops counters from running:

This is done unconditionally in book3s_hv_interrupts.S upon entering the
guest, and the original value is only save/restored if the host has
indicated it was using the PMU. This is okay, however the user of the
PMU needs to ensure that it is in a defined state when it starts using
it.

We are seeing a lot of PMU warnings on POWER8:
Can't find PMC that caused IRQ
Looking closer, the active PMC is 0 at this point and we took a PMU
exception on the transition from negative to 0. Some versions of POWER8
have an issue where they edge detect and not level detect PMC overflows.
A number of places program the PMC with (0x80000000 - period_left),
where period_left can be negative. We can either fix all of these or
just ensure that period_left is always >= 1.
This patch takes the second option.

chips, advertise them to userspace using a specific bit to properly
differentiate with chips of the same architecture level that might not
have them.

commit 8f9c0119d7 (compat: fs: Generic compat_sys_sendfile
implementation) changed the PowerPC 64bit sendfile call from
sys_sendile64 to sys_sendfile.
Unfortunately this broke sendfile of lengths greater than 2G because
sys_sendfile caps at MAX_NON_LFS. Restore what we had previously which
fixes the bug.

The assembler maintainer says:
I changed the ABI, something that had to be done but unfortunately
happens to break the booke kernel code. When building up a 64-bit
value with lis, ori, shl, oris, ori or similar sequences, you now
should use @high and @higha in place of @h and @ha. @h and @ha
(and their associated relocs R_PPC64_ADDR16_HI and R_PPC64_ADDR16_HA)
now report overflow if the value is out of 32-bit signed range.
ie. @h and @ha assume you're building a 32-bit value. This is needed
to report out-of-range -mcmodel=medium toc pointer offsets in @toc@h
and @toc@ha expressions, and for consistency I did the same for all
other @h and @ha relocs.
Replacing @h with @high in one strategic location fixes the relocation
errors. This has to be done conditionally since the assembler either
supports @h or @high but not both.

When we fork/clone we currently don't copy any of the TM state to the new
thread. This results in a TM bad thing (program check) when the new process is
switched in as the kernel does a tmrechkpt with TEXASR FS not set. Also, since
R1 is from userspace, we trigger the bad kernel stack pointer detection. So we
end up with something like this:

The below fixes this by flushing the TM state before we copy the task_struct to
the clone. To do this we go through the tmreclaim patch, which removes the
checkpointed registers from the CPU and transitions the CPU out of TM suspend
mode. Hence we need to call tmrechkpt after to restore the checkpointed state
and the TM mode for the current task.
To make this fail from userspace is simply:

This moves tm_recheckpoint to a C function and moves the tm_restore_sprs into
that function. It then adds IRQ disabling over the trechkpt critical section.
It also sets the TEXASR FS in the signals code to ensure this is never set now
that we explictly write the TM sprs in tm_recheckpoint.

A number of situations currently require the heavyweight smp_mb(),
even though there is no need to order prior stores against later
loads. Many architectures have much cheaper ways to handle these
situations, but the Linux kernel currently has no portable way
to make use of them.
This commit therefore supplies smp_load_acquire() and
smp_store_release() to remedy this situation. The new
smp_load_acquire() primitive orders the specified load against
any subsequent reads or writes, while the new smp_store_release()
primitive orders the specifed store against any prior reads or
writes. These primitives allow array-based circular FIFOs to be
implemented without an smp_mb(), and also allow a theoretical
hole in rcu_assign_pointer() to be closed at no additional
expense on most architectures.
In addition, the RCU experience transitioning from explicit
smp_read_barrier_depends() and smp_wmb() to rcu_dereference()
and rcu_assign_pointer(), respectively resulted in substantial
improvements in readability. It therefore seems likely that
replacing other explicit barriers with smp_load_acquire() and
smp_store_release() will provide similar benefits. It appears
that roughly half of the explicit barriers in core kernel code
might be so replaced.

The 64bit relocation code places a few symbols in the text segment.
These symbols are only 4 byte aligned where they need to be 8 byte

In copy_oldmem_page, the current check using max_pfn and min_low_pfn to
decide if the page is backed or not, is not valid when the memory layout is
not continuous.
This happens when running as a QEMU/KVM guest, where RTAS is mapped higher
in the memory. In that case max_pfn points to the end of RTAS, and a hole
between the end of the kdump kernel and RTAS is not backed by PTEs. As a
consequence, the kdump kernel is crashing in copy_oldmem_page when accessing
in a direct way the pages in that hole.
This fix relies on the memblock's service memblock_is_region_memory to
check if the read page is part or not of the directly accessible memory.

Currently we're storing a host endian RTAS token in
rtas_stop_self_args.token. We then pass that directly to rtas. This is
fine on big endian however on little endian the token is not what we
expect.
This will typically result in hitting:
panic("Alas, I survived.\n");
To fix this we always use the stop-self token in host order and always
convert it to be32 before passing this to rtas.

This patch moves the generalized implementation of of_get_cpu_node from
PowerPC to DT core library, thereby adding support for retrieving cpu
node for a given logical cpu index on any architecture.
The CPU subsystem can now use this function to assign of_node in the
cpu device while registering CPUs.
It is recommended to use these helper function only in pre-SMP/early
initialisation stages to retrieve CPU device node pointers in logical
ordering. Once the cpu devices are registered, it can be retrieved easily
from cpu device of_node which avoids unnecessary parsing and matching.

Unlike global OOM handling, memory cgroup code will invoke the OOM killer
in any OOM situation because it has no way of telling faults occuring in
kernel context - which could be handled more gracefully - from
user-triggered faults.
Pass a flag that identifies faults originating in user space from the
architecture-specific fault handlers to generic code so that memcg OOM
handling can be improved.

Currently hugepage migration works well only for pmd-based hugepages
(mainly due to lack of testing,) so we had better not enable migration of
other levels of hugepages until we are ready for it.
Some users of hugepage migration (mbind, move_pages, and migrate_pages) do
page table walk and check pud/pmd_huge() there, so they are safe. But the
other users (softoffline and memory hotremove) don't do this, so without
this patch they can try to migrate unexpected types of hugepages.
To prevent this, we introduce hugepage_migration_support() as an
architecture dependent check of whether hugepage are implemented on a pmd
basis or not. And on some architecture multiple sizes of hugepages are
available, so hugepage_migration_support() also checks hugepage size.

On some PAE architectures, the entire range of physical memory could reside
outside the 32-bit limit. These systems need the ability to specify the
initrd location using 64-bit numbers.
This patch globally modifies the early_init_dt_setup_initrd_arch() function to
use 64-bit numbers instead of the current unsigned long.
There has been quite a bit of debate about whether to use u64 or phys_addr_t.
It was concluded to stick to u64 to be consistent with rest of the device
tree code. As summarized by Geert, "The address to load the initrd is decided
by the bootloader/user and set at that point later in time. The dtb should not
be tied to the kernel you are booting"

Concentrate code to modify totalram_pages into the mm core, so the arch
memory initialized code doesn't need to take care of it. With these
changes applied, only following functions from mm core modify global
variable totalram_pages: free_bootmem_late(), free_all_bootmem(),
free_all_bootmem_node(), adjust_managed_page_count().
With this patch applied, it will be much more easier for us to keep
totalram_pages and zone->managed_pages in consistence.

The code in remove_cache_dir() is supposed to remove the "cache"
subdirectory from the sysfs directory for a CPU when that CPU is
being offlined. It tries to do this by calling kobject_put() on
the kobject for the subdirectory. However, the subdirectory only
gets removed once the last reference goes away, and the reference
being put here may well not be the last reference. That means
that the "cache" subdirectory may still exist when the offlining
operation has finished. If the same CPU subsequently gets onlined,
the code tries to add a new "cache" subdirectory. If the old
subdirectory has not yet been removed, we get a WARN_ON in the
sysfs code, with stack trace, and an error message printed on the
console. Further, we ultimately end up with an online cpu with no
"cache" subdirectory.
This fixes it by doing an explicit kobject_del() at the point where
we want the subdirectory to go away. kobject_del() removes the sysfs
directory even though the object still exists in memory. The object
will get freed at some point in the future. A subsequent onlining
operation can create a new sysfs directory, even if the old object
still exists in memory, without causing any problems.

On POWER platforms, the hypervisor can notify the guest kernel about dynamic
changes in the cpu-numa associativity (VPHN topology update). Hence the
cpu-to-node mappings that we got from the firmware during boot, may no longer
be valid after such updates. This is handled using the arch_update_cpu_topology()
hook in the scheduler, and the sched-domains are rebuilt according to the new
mappings.
But unfortunately, at the moment, CPU hotplug ignores these updated mappings
and instead queries the firmware for the cpu-to-numa relationships and uses
them during CPU online. So the kernel can end up assigning wrong NUMA nodes
to CPUs during subsequent CPU hotplug online operations (after booting).
Further, a particularly problematic scenario can result from this bug:
On POWER platforms, the SMT mode can be switched between 1, 2, 4 (and even 8)
threads per core. The switch to Single-Threaded (ST) mode is performed by
offlining all except the first CPU thread in each core. Switching back to
SMT mode involves onlining those other threads back, in each core.
Now consider this scenario:
1. During boot, the kernel gets the cpu-to-node mappings from the firmware
and assigns the CPUs to NUMA nodes appropriately, during CPU online.
2. Later on, the hypervisor updates the cpu-to-node mappings dynamically and
communicates this update to the kernel. The kernel in turn updates its
cpu-to-node associations and rebuilds its sched domains. Everything is
fine so far.
3. Now, the user switches the machine from SMT to ST mode (say, by running
ppc64_cpu --smt=1). This involves offlining all except 1 thread in each
core.
4. The user then tries to switch back from ST to SMT mode (say, by running
ppc64_cpu --smt=4), and this involves onlining those threads back. Since
CPU hotplug ignores the new mappings, it queries the firmware and tries to
associate the newly onlined sibling threads to the old NUMA nodes. This
results in sibling threads within the same core getting associated with
different NUMA nodes, which is incorrect.
The scheduler's build-sched-domains code gets thoroughly confused with this
and enters an infinite loop and causes soft-lockups, as explained in detail
in commit 3be7db6ab (powerpc: VPHN topology change updates all siblings).
So to fix this, use the numa_cpu_lookup_table to remember the updated
cpu-to-node mappings, and use them during CPU hotplug online operations.
Further, we also need to ensure that all threads in a core are assigned to a
common NUMA node, irrespective of whether all those threads were online during
the topology update. To achieve this, we take care not to use cpu_sibling_mask()
since it is not hotplug invariant. Instead, we use cpu_first_sibling_thread()
and set up the mappings manually using the 'threads_per_core' value for that
particular platform. This helps us ensure that we don't hit this bug with any
combination of CPU hotplug and SMT mode switching.

were not correct. (off by one in some cases)

is only 4 byte aligned when it should be 8 byte aligned. Fix this
by adding an explicit alignment.
This fixes an issue where POWER7 little endian builds with
CONFIG_RELOCATABLE=y fail to boot.

In EXCEPTION_PROLOG_COMMON() we check to see if the stack pointer (r1)
is valid when coming from the kernel. If it's not valid, we die but
with a nice oops message.
Currently we allocate a stack frame (subtract INT_FRAME_SIZE) before we
check to see if the stack pointer is negative. Unfortunately, this
won't detect a bad stack where r1 is less than INT_FRAME_SIZE.
This patch fixes the check to compare the modified r1 with
-INT_FRAME_SIZE. With this, bad kernel stack pointers (including NULL
pointers) are correctly detected again.

Since kvmppc_hv_find_lock_hpte() is called from both virtmode and
realmode, so it can trigger the deadlock.
Suppose the following scene:
Two physical cpuM, cpuN, two VM instances A, B, each VM has a group of
vcpus.
If on cpuM, vcpu_A_1 holds bitlock X (HPTE_V_HVLOCK), then is switched
out, and on cpuN, vcpu_A_2 try to lock X in realmode, then cpuN will be
caught in realmode for a long time.
What makes things even worse if the following happens,
On cpuM, bitlockX is hold, on cpuN, Y is hold.
vcpu_B_2 try to lock Y on cpuM in realmode
vcpu_A_2 try to lock X on cpuN in realmode

CONFIG_CPU_FREQ_TABLE will be always enabled when cpufreq framework is used, as
cpufreq core depends on it. So, we don't need this CONFIG option anymore as it
is not configurable. Remove CONFIG_CPU_FREQ_TABLE and update its users.

Chapter 14 of Documentation/CodingStyle says:
The preferred form for passing a size of a struct is the following:
p = kmalloc(sizeof(*p), ...);
The alternative form where struct name is spelled out hurts
readability and introduces an opportunity for a bug when the pointer
variable type is changed but the corresponding sizeof that is passed
to a memory allocator is not.
This wasn't followed consistently in drivers/cpufreq, let's make it
more consistent by always following this rule.

The "index" field of struct cpufreq_frequency_table was never an
index and isn't used at all by the cpufreq core. It only is useful
for cpufreq drivers for their internal purposes.
Many people nowadays blindly set it in ascending order with the
assumption that the core will use it, which is a mistake.
Rename it to "driver_data" as that's what its purpose is. All of its
users are updated accordingly.

In pte_alloc_one(), pgtable_page_ctor() is passed an address that has
not been converted by page_address() to the newly allocated PTE page.
When the PTE is freed, __pte_free_tlb() calls pgtable_page_dtor()
with an address to the PTE page that has been converted by page_address().
The mismatch in the PTE's page address causes pgtable_page_dtor() to access
invalid memory, so resources for that PTE (such as the page lock) is not
properly cleaned up.
On PPC32, only SMP kernels are affected.
On PPC64, only SMP kernels with 4K page size are affected.
This bug was introduced by commit d614bb0412
"powerpc: Move the pte free routines from common header".
On a preempt-rt kernel, a spinlock is dynamically allocated for each
PTE in pgtable_page_ctor(). When the PTE is freed, calling
pgtable_page_dtor() with a mismatched page address causes a memory leak,
as the pointer to the PTE's spinlock is bogus.
On mainline, there isn't any immediately obvious symptoms, but the
problem still exists here.

We fixed an issue but an improved solution was later discussed after the patch
was merged.
Firstly, this patch doesn't handle the 64bit signals case, which could also hit
this issue (but has never been reported).
Secondly, the original patch isn't clear what MSR VSX should be set to. The
new approach below always clears the MSR VSX bit (to indicate no VSX is in the
context) and sets it only in the specific case where VSX is available (ie. when
VSX has been used and the signal context passed has space to provide the
state).
This reverts the original patch and replaces it with the improved solution. It
also adds a 64 bit version.

The VSX MSR bit in the user context indicates if the context contains VSX
state. Currently we set this when the process has touched VSX at any stage.
Unfortunately, if the user has not provided enough space to save the VSX state,
we can't save it but we currently still set the MSR VSX bit.
This patch changes this to clear the MSR VSX bit when the user doesn't provide
enough space. This indicates that there is no valid VSX state in the user
context.
This is needed to support get/set/make/swapcontext for applications that use
VSX but only provide a small context. For example, getcontext in glibc
provides a smaller context since the VSX registers don't need to be saved over
the glibc function call. But since the program calling getcontext may have
used VSX, the kernel currently says the VSX state is valid when it's not. If
the returned context is then used in setcontext (ie. a small context without
VSX but with MSR VSX set), the kernel will refuse the context. This situation
has been reported by the glibc community.

Commit fba2369e6c (mm: use vm_unmapped_area() on powerpc architecture)
has a bug in slice_scan_available() where we compare an unsigned long
(high_slices) against a shifted int. As a result, comparisons against
the top 32 bits of high_slices (representing the top 32TB) always
returns 0 and the top of our mmap region is clamped at 32TB
This also breaks mmap randomisation since the randomised address is
always up near the top of the address space and it gets clamped down
to 32TB.

We need add PE to its own PELTV. Otherwise, the errors originated
from the PE might contribute to other PEs. In the result, we can't
clear up the error successfully even we're checking and clearing
errors during access to PCI config space.

This fixes a typo in the code that saves the guest DSCR (Data Stream
Control Register) into the kvm_vcpu_arch struct on guest exit. The
effect of the typo was that the DSCR value was saved in the wrong place,
so changes to the DSCR by the guest didn't persist across guest exit
and entry, and some host kernel memory got corrupted.

The csum_partial_copy_generic() function saves the PowerPC non-volatile
r14, r15, and r16 registers for the main checksum-and-copy loop.
Unfortunately, it fails to restore them upon error exit from this loop,
which results in silent corruption of these registers in the presumably
rare event of an access exception within that loop.
This commit therefore restores these register on error exit from the loop.

The csum_partial_copy_generic() uses register r7 to adjust the remaining
bytes to process. Unfortunately, r7 also holds a parameter, namely the
address of the flag to set in case of access exceptions while reading
the source buffer. Lacking a quantum implementation of PowerPC, this
commit instead uses register r9 to do the adjusting, leaving r7's
pointer uncorrupted.
