strlen is often used incorectly to get the length of strings
defined at compile time. In these cases, the behavior can be
replicated with sizeof(X) - 1, which is calculated at compile
time rather than runtime, reducing overhead. I've created a
simple macro to replace these instances and applied it to all
the files compiled into the angler kernel.

范龙飞 reports that KASAN can report a use-after-free in __lock_acquire.
The reason is due to insufficient serialization in lo_release(), which
will continue to use the loop device even after it has decremented the
lo_refcnt to zero.
In the meantime, another process can come in, open the loop device
again as it is being shut down. Confusion ensues.

Don't pass a size larger than iov_len to kernel_sendmsg().
Otherwise it will cause a NULL pointer deref when kernel_sendmsg()
returns with rv < size.
DRBD as external module has been around in the kernel 2.4 days already.
We used to be compatible to 2.4 and very early 2.6 kernels,
we used to use
rv = sock_sendmsg(sock, &msg, iov.iov_len);
then later changed to
rv = kernel_sendmsg(sock, &msg, &iov, 1, size);
when we should have used
rv = kernel_sendmsg(sock, &msg, &iov, 1, iov.iov_len);
tcp_sendmsg() used to totally ignore the size parameter.
57be5bd ip: convert tcp_sendmsg() to iov_iter primitives
changes that, and exposes our long standing error.
Even with this error exposed, to trigger the bug, we would need to have
an environment (config or otherwise) causing us to not use sendpage()
for larger transfers, a failing connection, and have it fail "just at the
right time". Apparently that was unlikely enough for most, so this went
unnoticed for years.
Still, it is known to trigger at least some of these,
and suspected for the others:

It is correct for all stable branches older than that as well
(which contain the DRBD driver; which is 2.6.33 and up).

be bad when the value is in memory shared with the frontend.
When converting a non-native request to a native one, ensure that

Make the "Attempted send on closed socket" error messages generated in
nbd_request_handler() ratelimited.
When the nbd socket is shutdown, the nbd_request_handler() function emits
an error message for every request remaining in its queue. If the queue
is large, this will spam a large amount of messages to the log. There's
no need for a separate error message for each request, so this patch
ratelimits it.
In the specific case this was found, the system was virtual and the error
messages were logged to the serial port, which overwhelmed it.

Clear QUEUE_FLAG_ADD_RANDOM in all block drivers that set
QUEUE_FLAG_NONROT.
Historically, all block devices have automatically made entropy
contributions. But as previously stated in commit e2e1a148 ("block: add
sysfs knob for turning off disk entropy contributions"):
- On SSD disks, the completion times aren't as random as they
are for rotational drives. So it's questionable whether they
should contribute to the random pool in the first place.
- Calling add_disk_randomness() has a lot of overhead.
There are more reliable sources for randomness than non-rotational block
devices. From a security perspective it is better to err on the side of
caution than to allow entropy contributions from unreliable "random"
sources.

Do not __GFP_ZERO allocated zcomp ->private pages. We keep allocated
streams around and use them for read/write requests, so we supply a
zeroed out ->private to compression algorithm as a scratch buffer only
once -- the first time we use that stream. For the rest of IO requests
served by this stream ->private usually contains some temporarily data
from the previous requests.

Each zcomp backend uses own gfp flag but it's pointless because the
context they could be called is driven by upper layer(ie, zcomp
frontend). As well, zcomp frondend could call them in different
context. One context(ie, zram init part) is it should be better to make
sure successful allocation other context(ie, further stream allocation
part for accelarating I/O speed) is just optional so let's pass gfp down
from driver (ie, zcomp frontend) like normal MM convention.

When we're using LZ4 multi compression streams for zram swap, we found
out page allocation failure message in system running test. That was
not only once, but a few(2 - 5 times per test). Also, some failure
cases were continually occurring to try allocation order 3.
In order to make parallel compression private data, we should call
kzalloc() with order 2/3 in runtime(lzo/lz4). But if there is no order
2/3 size memory to allocate in that time, page allocation fails. This
patch makes to use vmalloc() as fallback of kmalloc(), this prevents
page alloc failure warning.
After using this, we never found warning message in running test, also
It could reduce process startup latency about 60-120ms in each case.

We can end up allocating a new compression stream with GFP_KERNEL from
within the IO path, which may result is nested (recursive) IO
operations. That can introduce problems if the IO path in question is a
reclaimer, holding some locks that will deadlock nested IOs.
Allocate streams and working memory using GFP_NOIO flag, forbidding
recursive IO and FS operations.

zram could kunmap_atomic() a NULL pointer in a rare situation: a zram
page becomes a full-zeroed page after a partial write io. The current
code doesn't handle this case and performs kunmap_atomic() on a NULL
pointer, which panics the kernel.
This patch fixes this issue.

xen-blkfront will crash if the check to talk_to_blkback()
in blkback_changed()(XenbusStateInitWait) returns an error.
The driver data is freed and info is set to NULL. Later during
the close process via talk_to_blkback's call to xenbus_dev_fatal()
the null pointer is passed to and dereference in blkfront_closing.

Mapping an image with a long parent chain (e.g. image foo, whose parent
is bar, whose parent is baz, etc) currently leads to a kernel stack
overflow, due to the following recursion in the reply path:
rbd_osd_req_callback()
rbd_obj_request_complete()
rbd_img_obj_callback()
rbd_img_parent_read_callback()
rbd_obj_request_complete()
...
Limit the parent chain to 16 images, which is ~5K worth of stack. When
the above recursion is eliminated, this limit can be lifted.

Currently we leak parent_spec and trigger a "parent reference
underflow" warning if rbd_dev_create() in rbd_dev_probe_parent() fails.
The problem is we take the !parent out_err branch and that only drops
refcounts; parent_spec that would've been freed had we called
rbd_dev_unparent() remains and triggers rbd_warn() in
rbd_dev_parent_put() - at that point we have parent_spec != NULL and
parent_ref == 0, so counter ends up being -1 after the decrement.
Redo rbd_dev_probe_parent() to fix this.

rbd requires stable pages, as it performs a crc of the page data before
they are send to the OSDs.
But since kernel 3.9 (patch 1d1d1a7672
"mm: only enforce stable page writes if the backing device requires
it") it is not assumed anymore that block devices require stable pages.
This patch sets the necessary flag to get stable pages back for rbd.
In a ceph installation that provides multiple ext4 formatted rbd
devices "bad crc" messages appeared regularly (ca 1 message every 1-2
minutes on every OSD that provided the data for the rbd) in the
OSD-logs before this patch. After this patch this messages are pretty
much gone (only ca 1-2 / month / OSD).

If rbd_dev_image_probe() in rbd_dev_probe_parent() fails, header_name
is freed twice: once in rbd_dev_probe_parent() and then in its caller
rbd_dev_image_probe() (rbd_dev_image_probe() is called recursively to
handle parent images).
rbd_dev_probe_parent() is responsible for probing the parent, so it
shouldn't muck with clone's fields.

3.15 upstream has many patches to zram that significantly improve
performance. Rebase zram and zsmalloc from that time.
zsmalloc: move it under mm
This patch moves zsmalloc under mm directory.
Before that, description will explain why we have needed custom
allocator.
Zsmalloc is a new slab-based memory allocator for storing compressed
pages. It is designed for low fragmentation and high allocation success
rate on large object, but <= PAGE_SIZE allocations.
zsmalloc differs from the kernel slab allocator in two primary ways to
achieve these design goals.
zsmalloc never requires high order page allocations to back slabs, or
"size classes" in zsmalloc terms. Instead it allows multiple
single-order pages to be stitched together into a "zspage" which backs
the slab. This allows for higher allocation success rate under memory
pressure.
Also, zsmalloc allows objects to span page boundaries within the zspage.
This allows for lower fragmentation than could be had with the kernel
slab allocator for objects between PAGE_SIZE/2 and PAGE_SIZE. With the
kernel slab allocator, if a page compresses to 60% of it original size,
the memory savings gained through compression is lost in fragmentation
because another object of the same size can't be stored in the leftover
space.
This ability to span pages results in zsmalloc allocations not being
directly addressable by the user. The user is given an
non-dereferencable handle in response to an allocation request. That
handle must be mapped, using zs_map_object(), which returns a pointer to
the mapped region that can be used. The mapping is necessary since the
object data may reside in two different noncontigious pages.
The zsmalloc fulfills the allocation needs for zram perfectly

zsmalloc: Fix CPU hotplug callback registration
Subsystems that want to register CPU hotplug callbacks, as well as perform
initialization for the CPUs that are already online, often do it as shown
below:

This is wrong, since it is prone to ABBA deadlocks involving the
cpu_add_remove_lock and the cpu_hotplug.lock (when running concurrently

Instead, the correct and race-free way of performing the callback

zram: promote zram from staging
Zram has lived in staging for a LONG LONG time and have been
fixed/improved by many contributors so code is clean and stable now. Of
course, there are lots of product using zram in real practice.
The major TV companys have used zram as swap since two years ago and
recently our production team released android smart phone with zram
which is used as swap, too and recently Android Kitkat start to use zram
for small memory smart phone. And there was a report Google released
their ChromeOS with zram, too and cyanogenmod have been used zram long
time ago. And I heard some disto have used zram block device for tmpfs.
In addition, I saw many report from many other peoples. For example,
Lubuntu start to use it.
The benefit of zram is very clear. With my experience, one of the
benefit was to remove jitter of video application with backgroud memory
pressure. It would be effect of efficient memory usage by compression
but more issue is whether swap is there or not in the system. Recent
mobile platforms have used JAVA so there are many anonymous pages. But
embedded system normally are reluctant to use eMMC or SDCard as swap
because there is wear-leveling and latency issues so if we do not use
swap, it means we can't reclaim anoymous pages and at last, we could
encounter OOM kill. :(
Although we have real storage as swap, it was a problem, too. Because
it sometime ends up making system very unresponsible caused by slow swap
storage performance.

zram: fix race between reset and flushing pending work
Dan and Sergey reported that there is a racy between reset and flushing
of pending work so that it could make oops by freeing zram->meta in
reset while zram_slot_free can access zram->meta if new request is
adding during the race window.
This patch moves flush after taking init_lock so it prevents new request
so that it closes the race.

zram: delay pending free request in read path
Sergey reported we don't need to handle pending free request every I/O
so that this patch removes it in read path while we remain it in write
path.
Let's consider below example.
Swap subsystem ask to zram "A" block free by swap_slot_free_notify but
zram had been pended it without real freeing. Swap subsystem allocates
"A" block for new data but request pended for a long time just handled
and zram blindly free new data on the "A" block. :(
That's why we couldn't remove handle pending free request right before
zram-write.

zram: use atomic operation for stat
Some of fields in zram->stats are protected by zram->lock which is
rather coarse-grained so let's use atomic operation without explict
locking.
This patch is ready for removing dependency of zram->lock in read path
which is very coarse-grained rw_semaphore. Of course, this patch adds
new atomic operation so it might make slow but my 12CPU test couldn't
spot any regression. All gain/lose is marginal within stddev.

zram: introduce zram->tb_lock
Currently, the zram table is protected by zram->lock but it's rather
coarse-grained lock and it makes hard for scalibility.
Let's use own rwlock instead of depending on zram->lock. This patch
adds new locking so obviously, it would make slow but this patch is just
prepartion for removing coarse-grained rw_semaphore(ie, zram->lock)
which is hurdle about zram scalability.
Final patch in this patchset series will remove the lock from read-path
and change rw_semaphore with mutex in write path. With bonus, we could
drop pending slot free mess in next patch.

zram: remove workqueue for freeing removed pending slot
Commit a0c516cbfc74 ("zram: don't grab mutex in zram_slot_free_noity")
introduced free request pending code to avoid scheduling by mutex under
spinlock and it was a mess which made code lenghty and increased
overhead.
Now, we don't need zram->lock any more to free slot so this patch
reverts it and then, tb_lock should protect it.

zram: remove zram->lock in read path and change it with mutex
Finally, we separated zram->lock dependency from 32bit stat/ table
handling so there is no reason to use rw_semaphore between read and
write path so this patch removes the lock from read path totally and
changes rw_semaphore with mutex. So, we could do

The below data proves mixed workload performs well 11 times and there is
also enhance on write-write path because current rw-semaphore doesn't
support SPIN_ON_OWNER. It's side effect but anyway good thing for us.
Write-related tests perform better (from 61% to 1058%) but read path has
good/bad(from -2.22% to 1.45%) but they are all marginal within stddev.

zram: do not pass rw argument to __zram_make_request()
Do not pass rw argument down the __zram_make_request() -> zram_bvec_rw()
chain, decode it in zram_bvec_rw() instead. Besides, this is the place
where we distinguish READ and WRITE bio data directions, so account zram
RW stats here, instead of __zram_make_request(). This also allows to
account a real number of zram READ/WRITE operations, not just requests
(single RW request may cause a number of zram RW ops with separate
locking, compression/decompression, etc).

zram: remove good and bad compress stats
Remove `good' and `bad' compressed sub-requests stats. RW request may
cause a number of RW sub-requests. zram used to account `good' compressed
sub-queries (with compressed size less than 50% of original size), `bad'
compressed sub-queries (with compressed size greater that 75% of original
size), leaving sub-requests with compression size between 50% and 75% of
original size not accounted and not reported. zram already accounts each
sub-request's compression size so we can calculate real device compression
ratio.

zram: use atomic64_t for all zram stats
This is a preparation patch for stats code duplication removal.
1) use atomic64_t for `pages_zero' and `pages_stored' zram stats.
2) `compr_size' and `pages_zero' struct zram_stats members did not
follow the existing device attr naming scheme: zram_stats.ATTR has
ATTR_show() function. rename them:
-- compr_size -> compr_data_size
-- pages_zero -> zero_pages

If we really have trouble with atomic stat operation, we could
change it with percpu_counter so that it could solve atomic overhead and
unnecessary memory space by introducing unsigned long instead of 64bit
atomic_t.

zram: delete zram_init_device()
allocate new `zram_meta' in disksize_store() only for uninitialised zram
device, saving a number of allocations and deallocations in case if
disksize_store() was called on currently used device. at the same time
zram_meta stack variable is not necessary, because we can set ->meta
directly. there is also no need in setting QUEUE_FLAG_NONROT queue on
every disksize_store(), set it once during device creation.

ZRAM performs direct LZO compression algorithm calls, making it the one
and only option. While LZO is generally performs well, LZ4 algorithm
tends to have a faster decompression (see http://code.google.com/p/lz4/

Thus, users who have mostly read (decompress) usage scenarious or mixed
workflow (writes with relatively high read ops number) will benefit from
using LZ4 compression backend.
Introduce compressing backend abstraction zcomp in order to support
multiple compression algorithms with the following set of operations:
.create
.destroy
.compress
.decompress
Schematically zram write() usually contains the following steps:
0) preparation (decompression of partioal IO, etc.)
1) lock buffer_lock mutex (protects meta compress buffers)
2) compress (using meta compress buffers)
3) alloc and map zs_pool object
4) copy compressed data (from meta compress buffers) to object allocated by 3)
5) free previous pool page, assign a new one
6) unlock buffer_lock mutex
As we can see, compressing buffers must remain untouched from 1) to 4),
because, otherwise, concurrent write() can overwrite data. At the same
time, zram_meta must be aware of a) specific compression algorithm memory
requirements and b) necessary locking to protect compression buffers. To
remove requirement a) new struct zcomp_strm introduced, which contains a
compress/decompress `buffer' and compression algorithm `private' part.
While struct zcomp implements zcomp_strm stream handling and locking and
removes requirement b) from zram meta. zcomp ->create() and ->destroy(),
respectively, allocate and deallocate algorithm specific zcomp_strm
`private' part.
Every zcomp has zcomp stream and mutex to protect its compression stream.
Stream usage semantics remains the same -- only one write can hold stream
lock and use its buffers. zcomp_strm_find() turns caller into exclusive
user of a stream (holding stream mutex until zram release stream), and
zcomp_strm_release() makes zcomp stream available (unlock the stream
mutex). Hence no concurrent write (compression) operations possible at
the moment.

zram: factor out single stream compression
This is preparation patch to add multi stream support to zcomp.
Introduce struct zcomp_strm_single and a set of functions to manage
zcomp_strm stream access. zcomp_strm_single implements single compession
stream, same way as current zcomp implementation. This moves zcomp_strm
stream control and locking from zcomp, so compressing backend zcomp is not
aware of required locking.
Single and multi streams require different locking schemes. Minchan Kim
reported that spinlock-based locking scheme (which is used in multi stream
implementation) has demonstrated a severe perfomance regression for single
compression stream case, comparing to mutex-based. see

The following set of functions added:
- zcomp_strm_single_find()/zcomp_strm_single_release()
find and release a compression stream, implement required locking

zram: add multi stream functionality
Existing zram (zcomp) implementation has only one compression stream
(buffer and algorithm private part), so in order to prevent data
corruption only one write (compress operation) can use this compression
stream, forcing all concurrent write operations to wait for stream lock
to be released. This patch changes zcomp to keep a compression streams
list of user-defined size (via sysfs device attr). Each write operation
still exclusively holds compression stream, the difference is that we
can have N write operations (depending on size of streams list)
executing in parallel. See TEST section later in commit message for
performance data.
Introduce struct zcomp_strm_multi and a set of functions to manage
zcomp_strm stream access. zcomp_strm_multi has a list of idle
zcomp_strm structs, spinlock to protect idle list and wait queue, making
it possible to perform parallel compressions.
The following set of functions added:
- zcomp_strm_multi_find()/zcomp_strm_multi_release()
find and release a compression stream, implement required locking
- zcomp_strm_multi_create()/zcomp_strm_multi_destroy()
create and destroy zcomp_strm_multi
zcomp ->strm_find() and ->strm_release() callbacks are set during
initialisation to zcomp_strm_multi_find()/zcomp_strm_multi_release()
correspondingly.
Each time zcomp issues a zcomp_strm_multi_find() call, the following set
of operations performed:
- spin lock strm_lock
- if idle list is not empty, remove zcomp_strm from idle list, spin
unlock and return zcomp stream pointer to caller
- if idle list is empty, current adds itself to wait queue. it will be
awaken by zcomp_strm_multi_release() caller.

- spin unlock, wake up sleeper

When only one compression stream available, mutex with spin on owner
tends to perform much better than frequent wait_event()/wake_up(). This
is why single stream implemented as a special case with mutex locking.
Introduce and document zram device attribute max_comp_streams. This
attr shows and stores current zcomp's max number of zcomp streams
(max_strm). Extend zcomp's zcomp_create() with `max_strm' parameter.
`max_strm' limits the number of zcomp_strm structs in compression
backend's idle list (max_comp_streams).
max_comp_streams used during initialisation as follows:
-- passing to zcomp_create() max_strm equals to 1 will initialise zcomp
using single compression stream zcomp_strm_single (mutex-based locking).
-- passing to zcomp_create() max_strm greater than 1 will initialise zcomp
using multi compression stream zcomp_strm_multi (spinlock-based locking).
default max_comp_streams value is 1, meaning that zram with single stream
will be initialised.
Later patch will introduce configuration knob to change max_comp_streams
on already initialised and used zcomp.

for zcomp_strm_multi and zcomp_strm_single, accordingly. set_max_streams
for single steam zcomp does nothing.
If user has lowered the limit, then zcomp_strm_multi_set_max_streams()
attempts to immediately free extra streams (as much as it can, depending
on idle streams availability).
Note, this patch does not allow to change stream 'policy' from single to
multi stream (or vice versa) on already initialised compression backend.

zram: add lz4 algorithm backend
Introduce LZ4 compression backend and make it available for selection.
LZ4 support is optional and requires user to set ZRAM_LZ4_COMPRESS config
option. The default compression backend is LZO.

zram: move comp allocation out of init_lock
While fixing lockdep spew of ->init_lock reported by Sasha Levin [1],
Minchan Kim noted [2] that it's better to move compression backend
allocation (using GPF_KERNEL) out of the ->init_lock lock, same way as
with zram_meta_alloc(), in order to prevent the same lockdep spew.

zram: return error-valued pointer from zcomp_create()
Instead of returning just NULL, return ERR_PTR from zcomp_create() if
compressing backend creation has failed. ERR_PTR(-EINVAL) for unsupported
compression algorithm request, ERR_PTR(-ENOMEM) for allocation (zcomp or
compression stream) error.
Perform IS_ERR() check of returned from zcomp_create() value in
disksize_store() and set return code to PTR_ERR().

zram: propagate error to user
When we initialized zcomp with single, we couldn't change
max_comp_streams without zram reset but current interface doesn't show
any error to user and even it changes max_comp_streams's value without
any effect so it would make user very confusing.
This patch prevents max_comp_streams's change when zcomp was initialized
as single zcomp and emit the error to user(ex, echo).

sysfs.txt documentation lists the following requirements:
- The buffer will always be PAGE_SIZE bytes in length. On i386, this
is 4096.
- show() methods should return the number of bytes printed into the
buffer. This is the return value of scnprintf().
- show() should always use scnprintf().

zram: support REQ_DISCARD
zram is ram based block device and can be used by backend of filesystem.
When filesystem deletes a file, it normally doesn't do anything on data
block of that file. It just marks on metadata of that file. This
behavior has no problem on disk based block device, but has problems on
ram based block device, since we can't free memory used for data block.
To overcome this disadvantage, there is REQ_DISCARD functionality. If
block device support REQ_DISCARD and filesystem is mounted with discard
option, filesystem sends REQ_DISCARD to block device whenever some data
blocks are discarded. All we have to do is to handle this request.
This patch implements to flag up QUEUE_FLAG_DISCARD and handle this
REQ_DISCARD request. With it, we can free memory used by zram if it isn't
used.

For write/discard obj_requests that involved a copyup method call, the
opcode of the first op is CEPH_OSD_OP_CALL and the ->callback is
rbd_img_obj_copyup_callback(). The latter frees copyup pages, sets
->xferred and delegates to rbd_img_obj_callback(), the "normal" image
object callback, for reporting to block layer and putting refs.
rbd_osd_req_callback() however treats CEPH_OSD_OP_CALL as a trivial op,
which means obj_request is marked done in rbd_osd_trivial_callback(),
*before* ->callback is invoked and rbd_img_obj_copyup_callback() has
a chance to run. Marking obj_request done essentially means giving
rbd_img_obj_callback() a license to end it at any moment, so if another
obj_request from the same img_request is being completed concurrently,
rbd_img_obj_end_request() may very well be called on such prematurally
marked done request:

Calling rbd_img_obj_end_request() on such a request leads to trouble,
in particular because its ->xfferred is 0. We report 0 to the block
layer with blk_update_request(), get back 1 for "this request has more
data in flight" and then trip on
rbd_assert(more ^ (which == img_request->obj_request_count));
with rhs (which == ...) being 1 because rbd_img_obj_end_request() has
been called for both requests and lhs (more) being 1 because we haven't
got a chance to set ->xfferred in rbd_img_obj_copyup_callback() yet.
To fix this, leverage that rbd wants to call class methods in only two
cases: one is a generic method call wrapper (obj_request is standalone)
and the other is a copyup (obj_request is part of an img_request). So
make a dedicated handler for CEPH_OSD_OP_CALL and directly invoke
rbd_img_obj_copyup_callback() from it if obj_request is part of an
img_request, similar to how CEPH_OSD_OP_READ handler invokes
rbd_img_obj_request_read_callback().
Since rbd_img_obj_copyup_callback() is now being called from the OSD
request callback (only), it is renamed to rbd_osd_copyup_callback().

rbd_obj_request_create() is called on the main I/O path, so we need to
use GFP_NOIO to make sure allocation doesn't blow back on us. Not all
callers need this, but I'm still hardcoding the flag inside rather than
making it a parameter because a) this is going to stable, and b) those
callers shouldn't really use rbd_obj_request_create() and will be fixed
in the future.
More memory allocation fixes will follow.

When we end I/O struct request with error, we need to pass
obj_request->length as @nr_bytes so that the entire obj_request worth
of bytes is completed. Otherwise block layer ends up confused and we
trip on
rbd_assert(more ^ (which == img_request->obj_request_count));
in rbd_img_obj_callback() due to more being true no matter what. We
already do it in most cases but we are missing some, in particular
those where we don't even get a chance to submit any obj_requests, due
to an early -ENOMEM for example.
A number of obj_request->xferred assignments seem to be redundant but
I haven't touched any of obj_request->xferred stuff to keep this small
and isolated.

Olivier Bonvalet reported having repeated crashes due to a failed
assertion he was hitting in rbd_img_obj_callback():
Assertion failure in rbd_img_obj_callback() at line 2165:
rbd_assert(which >= img_request->next_completion);
With a lot of help from Olivier with reproducing the problem
we were able to determine the object and image requests had
already been completed (and often freed) at the point the
assertion failed.
There was a great deal of discussion on the ceph-devel mailing list
about this. The problem only arose when there were two (or more)
object requests in an image request, and the problem was always
seen when the second request was being completed.
The problem is due to a race in the window between setting the
"done" flag on an object request and checking the image request's
next completion value. When the first object request completes, it
checks to see if its successor request is marked "done", and if
so, that request is also completed. In the process, the image
request's next_completion value is updated to reflect that both
the first and second requests are completed. By the time the
second request is able to check the next_completion value, it
has been set to a value *greater* than its own "which" value,
which caused an assertion to fail.
Fix this problem by skipping over any completion processing
unless the completing object request is the next one expected.
Test only for inequality (not >=), and eliminate the bad
assertion.

The VD_OP_GET_VTOC operation will succeed only if the vdisk backend has a
VTOC label, otherwise it will fail. In particular, it will return error
48 (ENOTSUP) if the disk has an EFI label. VTOC disk labels are already
handled by directly reading the disk in block/partitions/sun.c (enabled by
CONFIG_SUN_PARTITION which defaults to y on SPARC). Since port->label is
unused in the driver, remove the call and the field.

vio_dring_avail() will allow use of every dring entry, but when the last
entry is allocated then dr->prod == dr->cons which is indistinguishable from
the ring empty condition. This causes the next allocation to reuse an entry.
When this happens in sunvdc, the server side vds driver begins nack'ing the
messages and ends up resetting the ldc channel. This problem does not effect
sunvnet since it checks for < 2.
The fix here is to just never allocate the very last dring slot so that full
and empty are not the same condition. The request start path was changed to
check for the ring being full a bit earlier, and to stop the blk_queue if
there is no space left. The blk_queue will be restarted once the ring is
only half full again. The number of ring entries was increased to 512 which
matches the sunvnet and Solaris vdc drivers, and greatly reduces the
frequency of hitting the ring full condition and the associated blk_queue
stop/starting. The checks in sunvent were adjusted to account for
vio_dring_avail() returning 1 less.

ldc_map_sg() could fail its check that the number of pages referred to
by the sg scatterlist was <= the number of cookies.
This fixes the issue by doing a similar thing to the xen-blkfront driver,
ensuring that the scatterlist will only ever contain a segment count <=
port->ring_cookies, and each segment will be page aligned, and <= page
size. This ensures that the scatterlist is always mappable.

The LDom diskserver doesn't return reliable geometry data. In addition,
the types for all fields in the vio_disk_geom are u16, which were being
truncated in the cast into the u8's of the Linux struct hd_geometry.
Modify vdc_getgeo() to compute the geometry from the disk's capacity in a
manner consistent with xen-blkfront::blkif_getgeo().

Interpret the media type from v1.1 protocol to support CDROM/DVD.
For v1.0 protocol, a disk's size continues to be calculated from the
geometry returned by the vdisk server. The geometry returned by the server
can be less than the actual number of sectors available in the backing
image/device due to the rounding in the division used to compute the
geometry in the vdisk server.
In v1.1 protocol a disk's actual size in sectors is returned during the
handshake. Use this size when v1.1 protocol is negotiated. Since this size
will always be larger than the former geometry computed size, disks created
under v1.0 will be forwards compatible to v1.1, but not vice versa.

When we fail to allocate page vector in rbd_obj_read_sync() we just
basically ignore the problem and continue which will result in an oops
later. Fix the problem by returning proper error.

Commit 98683650 "Merge branch 'drbd-8.4_ed6' into
for-3.8-drivers-drbd-8.4_ed6" switches to the new augment API, but the
new API requires that the tree is augmented before rb_insert_augmented()
is called, which is missing.
So we add the augment-code to drbd_insert_interval() when it travels the
tree up to down before rb_insert_augmented(). See the example in
include/linux/interval_tree_generic.h or Documentation/rbtree.txt.
drbd_insert_interval() may cancel the insertion when traveling, in this
case, the just added augment-code does nothing before cancel since the
@this node is already in the subtrees in this case.

Since linux kernel 3.13, kthread_run() internally uses
wait_for_completion_killable(). We sometimes may use kthread_run()
while we still have a signal pending, which we used to kick our threads
out of potentially blocking network functions, causing kthread_run() to
mistake that as a new fatal signal and fail.
Fix: flush_signals() before kthread_run().

In addition to bringing in upstream commits, this merge also makes minor
changes to mainitain compatibility with upstream:
The definition of list_next_entry in qcrypto.c and ipa_dp.c has been
removed, as upstream has moved the definition to list.h. The implementation
of list_next_entry was identical between the two.
irq.c, for both arm and arm64 architecture, has had its calls to
__irq_set_affinity_locked updated to reflect changes to the API upstream.
Finally, as we have removed the sleep_length member variable of the
tick_sched struct, all changes made by upstream commit ec804bd do not
apply to our tree and have been removed from this merge. Only
kernel/time/tick-sched.c is impacted.

The following check in rbd_img_obj_request_submit()
rbd_dev->parent_overlap <= obj_request->img_offset
allows the fall through to the non-layered write case even if both
parent_overlap and obj_request->img_offset belong to the same RADOS
object. This leads to data corruption, because the area to the left of
parent_overlap ends up unconditionally zero-filled instead of being
populated with parent data. Suppose we want to write 1M to offset 6M
of image bar, which is a clone of foo@snap; object_size is 4M,
parent_overlap is 5M:

parent_overlap obj_request->img_offset
4..5M should be copyup'ed from foo, yet it is zero-filled, just like
5..6M is.
Given that the only striping mode kernel client currently supports is
chunking (i.e. stripe_unit == object_size, stripe_count == 1), round
parent_overlap up to the next object boundary for the purposes of the
overlap check.

Each image request contains a reference count, but to date it has
not actually been used. (I think this was just an oversight.) A
recent report involving rbd failing an assertion shed light on why
and where we need to use these reference counts.
Every OSD request associated with an object request uses
rbd_osd_req_callback() as its callback function. That function will
call a helper function (dependent on the type of OSD request) that
will set the object request's "done" flag if the object request if
appropriate. If that "done" flag is set, the object request is
passed to rbd_obj_request_complete().
In rbd_obj_request_complete(), requests are processed in sequential
order. So if an object request completes before one of its
predecessors in the image request, the completion is deferred.
Otherwise, if it's a completing object's "turn" to be completed, it
is passed to rbd_img_obj_end_request(), which records the result of
the operation, accumulates transferred bytes, and so on. Next, the
successor to this request is checked and if it is marked "done",
(deferred) completion processing is performed on that request, and
so on. If the last object request in an image request is completed,
rbd_img_request_complete() is called, which (typically) destroys
the image request.
There is a race here, however. The instant an object request is
marked "done" it can be provided (by a thread handling completion of
one of its predecessor operations) to rbd_img_obj_end_request(),
which (for the last request) can then lead to the image request
getting torn down. And this can happen *before* that object has
itself entered rbd_img_obj_end_request(). As a result, once it
*does* enter that function, the image request (and even the object
request itself) may have been freed and become invalid.
All that's necessary to avoid this is to properly count references
to the image requests. We tear down an image request's object
requests all at once--only when the entire image request has
completed. So there's no need for an image request to count
references for its object requests. However, we don't want an
image request to go away until the last of its object requests
has passed through rbd_img_obj_callback(). In other words,
we don't want rbd_img_request_complete() to necessarily
result in the image request being destroyed, because it may
get called before we've finished processing on all of its
object requests.
So the fix is to add a reference to an image request for
each of its object requests. The reference can be viewed
as representing an object request that has not yet finished
its call to rbd_img_obj_callback(). That is emphasized by
getting the reference right after assigning that as the image
object's callback function. The corresponding release of that
reference is done at the end of rbd_img_obj_callback(), which
every image object request passes through exactly once.

In module exit, dfs_parent and it's subtree were removed before
unregistering with pci. When debugfs entry for each device is attempted
to remove in pci_remove() context, they don't exist, as dfs_parent and
its children were already ripped apart.
Modified to first unregister with pci and then remove dfs_parent.
