This fixes a pretty ancient bug that hasn't manifested itself
until now.
The scratchbuf for command queue is allocated only for 32 slots
but is accessed with the queue write pointer - which can be
up to 256.
Since the scratch buf size was 16 and there are up to 256 TFDs
we never passed a page boundary when accessing the scratch buffer,
but when attempting to increase the size of the scratch buffer a
panic was quick to follow when trying to access the address resulted
in a page boundary.

The assumption before this patch was that we don't need to
run again the INIT firmware after the system booted. The
INIT firmware runs calibrations which impact the physical
layer's behavior.
Users reported that it may be helpful to run these
calibrations again every time the interface is brought up.
The penatly is minimal, since the calibrations run fast.

We don't really need to use different mac colors when adding mac
contexts, because they're not used anywhere. In fact, the firmware
doesn't accept 255 as a valid color, so we get into a SYSASSERT 0x3401
when we reach that.
Remove the color increment to use always zero and avoid reaching 255.

The base address of the scheduler in the device's memory
(SRAM) comes from two different sources. The periphery
register and the alive notification from the firmware.
We have a check in iwl_pcie_tx_start that ensures that
they are the same.
When we resume from WoWLAN, the firmware may have crashed
for whatever reason. In that case, the whole device may be
reset which means that the periphery register will hold a
meaningless value. When we come to compare
trans_pcie->scd_base_addr (which really holds the value we
had when we loaded the WoWLAN firmware upon suspend) and
the current value of the register, we don't see a match
unsurprisingly.
Trick the check to avoid a loud yet harmless WARN.
Note that when the WoWLAN has crashed, we will see that
in iwl_trans_pcie_d3_resume which will let the op_mode
know. Once the op_mode is informed that the WowLAN firmware
has crashed, it can't do much besides resetting the whole
device.

The LTR is the handshake between the device and the root
complex about the latency allowed when the bus exits power
save. This configuration was missing and this led to high
latency in the link power up. The end user could experience
high latency in the network because of this.

We should always prefer to use full RTS protection. Using
CTS to self gives a meaningless improvement, but this flow
is much harder for the firmware which is likely to have
issues with it.

Since the statistics handler is asynchrous, it can very well
be that we will handle the statistics (hence the RSSI
fluctuation) when we already disassociated.

NICs supported by iwldvm don't handle well TX AMPDU.
Disable it by default, still leave the possibility to
the user to force enable it with a debug parameter.
NICs supported by iwlmvm don't suffer from the same issue,
leave TX AMPDU enabled by default for these.

Only the first packet is currently handled correctly, but then
all others are assumed to have failed which is problematic. Fix
this, marking them all successful instead (since if they're not
then the firmware will have transmitted them as single frames.)
This fixes the lost packet reporting.
Also do a tiny variable scoping cleanup.

We set IWL_STA_UCODE_INPROGRESS flag when we add a station
and clear it when we send the LQ command for it. But the LQ
command is sent only when the association succeeds.
If the association doesn't succeed, we would leave this flag
set and that wouldn't indicate the station entry as vacant.

The driver wasn't reading the NVM properly. While this
didn't lead to any issue until now, it seems that there
is an old version of the NVM in the wild.
In this version, the A band channels appear to be valid
but the SKU capabilities (another field of the NVM) says
that A band isn't supported at all.
With this specific version of the NVM, the driver would
think that A band is supported while the HW / firmware
don't. This leads to asserts.

We changed the timeout for the interrupt coealescing for
calibration, but that wasn't effective since we changed
that value back before loading the firmware. Since
calibrations are notification from firmware and not Rx
packets, this doesn't change anyway - the firmware will
fire an interrupt straight away regardless of the interrupt
coalescing value.
Also, a HW issue has been discovered in 7000 devices series.
The work around is to disable the new interrupt coalescing
timeout feature - do this by setting bit 31 in
CSR_INT_COALESCING.
This has been fixed in 7265 which means that we can't rely
on the device family and must have a hint in the iwl_cfg
structure.

Enabling the oscillator consumes slightly more power (100uA)
but allows to make sure that we exit from L1 on time.
Not doing so might lead to a PCIe specification violation
since we might wake up from L1 at the wrong time.
This issue has been identified on 3160 and 7260 only.
On older NICs L1 off is not enabled, on newer NICs (7265),
the issue is fixed.
When the bug occurs the user sees that the NIC has
disappeared from the PCI bridge, any access to the device
returns 0xff.

Since we set IEEE80211_HW_QUEUE_CONTROL, we can let
mac80211 do the queue assignement and don't need to
override its decisions.
While reassiging the same values is harmless of course,
it triggered a WARNING when iwlwifi and mac80211 came
to different conclusions. This happened when mac80211 set
IEEE80211_TX_CTL_SEND_AFTER_DTIM, but didn't route the
packet to the cab_queue because no stations were asleep.
iwlwifi should not override mac80211's decicions for
offchannel packets and packets to be sent after DTIM,
but it should override mac80211's decision for AMPDUs
since we have a special queue for them. So for AMPDU,
we still override info->hw_queue by the AMPDU queue.
This avoids:

This is because we nulify ctx->vif in iwlagn_mac_remove_interface()
before calling some other functions that teardown interface. To fix
just check ctx->vif on iwl_chswitch_done(). We should not call
ieee80211_chswitch_done() as channel switch works were already canceled
by mac80211 in ieee80211_do_stop() -> ieee80211_mgd_stop().

As the firmware API has changed significantly and we don't
have support code for the old APIs, bump the version to be
able to release the version 7 API firmware. Unfortunately
this means that the driver in 3.9 and 3.10 can't work, but
that's still better than crashing the device/driver there.

The MCAST queue should be enabled after DTIM only.
According to fw API, the MCAST must not be attached to any
station, but should appear in the mcast_qid of the AP's
mac context only.

In multicast, there is no retries nor RTS since there is no
specific recipient that can ACK or send CTS. This means
that we must not use the rate scale table for multicast
frames.
This true for any frame that doesn't have a valid
ieee80211_sta pointer.

The AP mode needs to use the MCAST fifo for the MCAST
frames sent after the DTIM. This fifo needs to be
configured with the same parameters as the VOICE FIFO.
A separate SCD queue is mapped to this fifo - the cab_queue
(cab stands for Content After Beacon). This queue isn't
connected to any station, but rather to the MAC context.
This queue should (and is already) be set as the MCAST
queue - this is part of the of MAC context command.

The BT_CONFIG command that is sent to the device during
startup will enable BT coex unless the module parameter
turns it off, but on devices without Bluetooth this may
cause problems, as reported in Redhat BZ 885407.
Fix this by sending the BT_CONFIG command only when the
device has Bluetooth.

Set SSID bitmap for direct scan even on passive channels,
for the passive-to-active feature. Without this patch only
the SSID from probe request template is sent on passive
channels, after passive-to-active switching, causing us to
not find all desired networks.
Remove the unused passive scan mask constant.

Due to a firmware bug, it crashes when the beacon interval
is smaller than 16. Avoid this by refusing the station state
change creating the AP station, causing mac80211 to abandon
the attempt to connect to the AP, and eventually wpa_s to
blacklist it.

Increment index in each iteration. Without this increment we are
overriding the added SSIDs and we will send only the last SSId
and (n_ssids - 1) broadcast probes.

When the queue is unmapped while it was so loaded that
mac80211's was stopped, we need to wake the queue after
having freed all the packets in the queue.
Not doing so can result in weird stuff like:
* run lots of traffic (mac80211's queue gets stopped)
* RFKILL
* de-assert RFKILL
* no traffic

When a queue is disabled, it frees all its entries. Later,
the op_mode might still get notifications from the firmware
that triggers to free entries in the tx queue. The transport
should be prepared for these races and know to ignore
reclaim calls on queues that have been disabled and whose
entries have been freed.
