Christoph Hellwig 1dff8de4ce 3w-xxxx: fix command completion race

The 3w-xxxx driver needs to tear down the dma mappings before returning
the command to the midlayer, as there is no guarantee the sglist and
count are valid after that point.  Also remove the dma mapping helpers
which have another inherent race due to the request_id index.

James Bottomley 62d37cc411 mvsas: fix panic on expander attached SATA devices
commit 56cbd0ccc1b508de19561211d7ab9e1c77e6b384 upstream.

mvsas is giving a General protection fault when it encounters an expander
attached ATA device.  Analysis of mvs_task_prep_ata() shows that the driver is
assuming all ATA devices are locally attached and obtaining the phy mask by
indexing the local phy table (in the HBA structure) with the phy id.  Since
expanders have many more phys than the HBA, this is causing the index into the
HBA phy table to overflow and returning rubbish as the pointer.

mvs_task_prep_ssp() instead does the phy mask using the port properties.
Mirror this in mvs_task_prep_ata() to fix the panic.

Merge commit 'v3.10.73' into LA.BF64.1.2.9

This merge brings us up to date with upstream kernel.org tag v3.10.73.
As part of the conflict resolution, changes introduced by commit 72684eae7
("arm64: Fix up /proc/cpuinfo") have been intentionally dropped, as they
conflict with Android changes msm-3.10 kernel to solve the problems
in a different way. Since userspace readers of this file may depend on
the existing msm-3.10 implementation, it's left as-is for now. The
commit may later be introduced if it is found to not impact userspaces
paired with this kernel.

Gilad Broner 4c63de9462 scsi: ufs: fix probe failure masked by crypto engine reset
When ufshcd_host_reset_and_restore() is called, it is possible
that ufshcd_probe_hba() fails and return an error value.
However, the later call to crypto_engine_reset() might succeed
and override the previous error value.
This will result in ufshcd_host_reset_and_restore() returning
success while in fact a serious error may have occured which
should prevent us from continuing in the normal sequence.
Instead, call crypto_engine_reset() only if ufshcd_probe_hba()
was successfull.

This patch fixes the incorrect use of __transport_register_session()
in tcm_qla2xxx_check_initiator_node_acl() code, that does not perform
explicit se_tpg->session_lock when accessing se_tpg->tpg_sess_list
to add new se_sess nodes.

Given that tcm_qla2xxx_check_initiator_node_acl() is not called with
qla_hw->hardware_lock held for all accesses of ->tpg_sess_list, the
code should be using transport_register_session() instead.

Gilad Broner fc991202bb scsi: ufs: implement scsi host timeout handler
A race condition exists between request requeueing and scsi layer
error handling:
When UFS driver queuecommand returns a busy status for a request,
it will be requeued and its tag will be freed and set to -1.
At the same time it is possible that the request will timeout and
scsi layer will start error handling for it. The scsi layer reuses
the request and its tag to send error related commands to the device,
however its tag is no longer valid.
As this request was never really sent to the device, there is no
point to start error handling with the device.
Implement the scsi error handling timeout callback and bypass SCSI
error handling for request that were not actually sent to the device.
For such requests simply reset the block layer timer. Otherwise, let
SCSI layer perform the usual error handling.

It's directly caused by 89d3cf6 [SCSI] libsas: add mutex for SMP task
execution, but shows a deeper cause: expander functions expect to be able to
cast to and treat domain devices as expanders.  The correct fix is to only do
expander discover when we know we've got an expander device to avoid wrongly
casting a non-expander device.

When running benchmarks on a system and inspecting the effect of
performance related changes it can be useful to know in a
simple way how long it takes for UFS requests to complete.
Add statistics for UFS requests completion times and measure
minimum, maximum and average completion times for all requests
and also according to request type: read, write and flush.

rq_data_dir(rq) will always be either READ or WRITE so conditional
logic could never classify a request as a flush request causing
flush statistics to always be zero.
In addition it is neccesary to check the request type is FS in
order not to count other request type as read/write.

Disabling requeueing of a completed request will cause a request
not to be retried (by requeueing it) in cases where the low-level
driver detected some error but finished processing of the requeust.
Specifically, not retrying the request will cause calls to functions
such as blk_execute_rq to block forever.

Brian King 7f8e9beb90 ipr: wait for aborted command responses
commit 6cdb08172bc89f0a39e1643c5e7eab362692fd1b upstream.

Fixes a race condition in abort handling that was injected
when multiple interrupt support was added. When only a single
interrupt is present, the adapter guarantees it will send
responses for aborted commands prior to the response for the
abort command itself. With multiple interrupts, these responses
generally come back on different interrupts, so we need to
ensure the abort thread waits until the aborted command is
complete so we don't perform a double completion. This race
condition was being hit frequently in environments which
were triggering command timeouts, which was resulting in
a double completion causing a kernel oops.

Long Li 14059bc870 storvsc: ring buffer failures may result in I/O freeze

When ring buffer returns an error indicating retry, storvsc may not
return a proper error code to SCSI when bounce buffer is not used.
This has introduced I/O freeze on RAID running atop storvsc devices.

Gilad Broner 5eb1a1a051 scsi: ufs: add host state prints in failure cases
Whenever some UFS failure occurs the driver prints the UFS
registers in order to help with analysis of the failure.
However this may not be sufficient in some cases, so having
the host controller state as it is represented and managed in
the driver will contribute to analysis efforts.
Added prints of various fields in the hba struct which may be
of interest.

For the long sequential tests, sector_range will define the size of the
sequential I/O to submit. For the long random tests, sector_range will
define the range in which to submit random I/O as well as the total size
of the random I/O which is defined by the combination of sector_range and
LONG_RAND_TEST_REQ_RATIO.

Unit tests submit large requests of 512KB made of 128 bios.
Current allocation was done via kmalloc which may not be able
to allocate such a large buffer which is also physically contiguous.
Using kmalloc to allocate each bio separately is also problematic as
it might not be page aligned. Some bio may end up using more than a
single memory segment which will fail the mapping of the bios to
the request which supports up to 128 physical segments.
To avoid such failure, allocate a separate page for each bio
(bio size is single page size).

Multi-query test fails on write descriptor requests becuase it
tries to write to the unit descriptor which is read-only according
to the spec.
The only writeable descriptors are the configuration and OEM ID
string descriptors which will be non-trivial to use.
Avoid sending write descriptor query for the time being, and replace
it with a read descriptor operation.

On some device, in some cases, the initialization time is
uncharacteristically long. This is within the standard which states
in section 13.4.6.1.2 (Dynamic Capacity Procedural Flow):

initialization..."
Therefore, making the loop pending on fDeviceInit to clear
longer.

ice_init may need UFS clocks to be enabled in order to allow
enabling ICE as part of its initialization. To allow that,
ice_init should be done as part of UFS initialization sequence.
In such a case ice_reset is no longer needed to be called as part of
UFS initialization and should be moved to host reset scenarios.

Maurizio Lombardi 8e3c025ae9 bnx2fc: do not add shared skbs to the fcoe_rx_list

In some cases, the fcoe_rx_list may contains multiple instances
of the same skb (the so called "shared skbs").

the bnx2fc_l2_rcv thread is a loop that extracts a skb from the list,
modifies (and destroys) its content and then proceed to the next one.
The problem is that if the skb is shared, the remaining instances will
be corrupted.

The solution is to use skb_share_check() before adding the skb to the
fcoe_rx_list.

Subhash Jadavani cee83d832c scsi: ufs: avoid exception event handler racing with PM callbacks
If device raises the exception event in the response to the commands
sent during the runtime/system PM callbacks, exception event handler
might run in parallel with PM callbacks and may see unclocked register
accesses. This change fixes this issue by not scheduling the exception
event handler while PM callbacks are running.

Consider following sequence of events:
1. UFS is runtime suspended, link_state = Hibern8, device_state = sleep
2. System goes into system suspend, ufshcd_system_suspend() brings both
   link and device to active state and then puts the device in Power_Down
   state and link in OFF state.
3. System resumes at some later point in time, ufshcd_system_resume()
   doesn't do anything as UFS state is runtime suspended. Note that link
   is still on OFF state and device is in Power_Down state.
4. Now system again goes into suspend without any UFS accesses before it.
   ufshcd_system_suspend() again brings both link and device to active
   state and then puts the device in Power_Down state and link if OFF
   state. But it's unnecessary to bring the link & device in active state
   as both link and device are already in desired low power states. This
   change fixes this issue by adding proper state checks in
   ufshcd_system_suspend().

Gilad Broner 5cdf5793c6 scsi: ufs: fix locking in __ufshcd_pm_qos_hold
The function is called with locked spinlock and disabled interrupts
which is done in a wrapper function ufshcd_pm_qos_hold().
Before flushing the PM QoS voting work spinlock must be released
and interrupts must be restored. As the interrupts flags state is
saved in the wrapper function, we cannot restore the state.
Moving the locking to the internal function fixes this, which
makes the wrapper function redundant so it is deleted.

Gilad Broner 799f085d62 scsi: ufs: adjust DME command delay
DME command delay is used to ensure a sufficient interval between
subsequent commands. The hardware programming guide specified 1ms.
Value of 700us was used as optimization, but it seems to be too short
so DME commands would sometimes fail.
Update the command delay to 1ms as recommended.

ufshcd_queuecommand() vote for the resources in this order: clocks,
pm_qos latency, hibern8 exit. If any of these votes are not already
applied, each one has to be applied asynchronously and in that case we are
releasing all the previously applied resource votes (for example, if
hibern8 exit has to be completed asynchronously, we release the votes for
pm_qos and clocks as well). This is not a optimal solution instead we
should skip scheduling the unvoting work for already voted resources.

Subhash Jadavani 7a9ab20920 scsi: ufs: fix possible unclocked register access during hibern8 exit
ufshcd_queuecommand() function calls ufshcd_hibern8_hold() to bring the
link out of hibern8 asynchronously. If the link was in hibern8 state then
it schedules a work to execute the hibern8 without any timeout and returns
error back to ufshcd_queuecommand(). ufshcd_queuecommand() calls
ufshcd_release() which schedules the clock gating work to be executed
after clock gating timeout (50ms at this moment). Ideally hibern8 exit
work should have finished much before clock gating work starts execution
but it may not be true on heavily loaded system. To protect this race,
hibern8 exit work should make sure that clocks are running (by calling
ufshcd_hold()) before executing hibern8 exit.

Tomas Henzl 2e4ce49881 SCSI: hpsa: fix a race in cmd_free/scsi_done

When the driver calls scsi_done and after that frees it's internal
preallocated memory it can happen that a new job is enqueud before
the memory is freed. The allocation fails and the message
"cmd_alloc returned NULL" is shown.
Patch below fixes it by moving cmd->scsi_done after cmd_free.

Christoph Hellwig 945f341afb scsi: only re-lock door after EH on devices that were reset

Setups that use the blk-mq I/O path can lock up if a host with a single
device that has its door locked enters EH.  Make sure to only send the
command to re-lock the door to devices that actually were reset and thus
might have lost their state.  Otherwise the EH code might be get blocked
on blk_get_request as all requests for non-reset devices might be in use.

Subhash Jadavani 9e990307b6 scsi: pm: keep request queue and scsi device runtime status in sync
If scsi device is not runtime suspended while system resumes then
scsi device's runtime PM status gets changed to RPM_ACTIVE (if its
parent status is already set to RPM_ACTIVE) by the scsi system resume
callback. But if the scsi device's runtime PM is managed by block layer
then runtime status of scsi device's request queue might still remain
in RPM_SUSPENDED hence all the new block requests submitted to the
request queue will have to wait until the queue runtime status gets
changed to RPM_ACTIVE which never happens as scsi device status is
already marked RPM_ACTIVE.

Fix this problem by changing the request queue runtime status to
RPM_ACTIVE when scsi device runtime status changes to RPM_ACTIVE.

target as there can be several requests that are aborted at once.

Gilad Broner 5eb07413cf scsi: ufs: suspend clock scaling for failed runtime_resume
During runtime resume operation, clock scaling may get indirectly
resumed via call to ufshcd_set_dev_pwr_mode(): Start/Stop Unit command
times out and SCSI error handling ultimately calls the host reset handler
to recover, during which clock scaling is resumed.
Error case exit path of runtime resume will disable clocks.
As clock scaling was already resumed, it will get scheduled later on and
try to access UFS registers while clocks are disabled, resulting in NOC
error.

Maya Erez 7600574ac1 scsi: ufs: Clear the aborted tag in case of ICE error
ICE error is handled as a fatal error, hence all the outstanding
requests are being aborted.
This patch clears the tag in case of device management requests
and fixes a bug where outstanding_tasks was cleared instead of
outstanding_reqs.

Subhash Jadavani c891d2c473 scsi: ufs_quirks: remove no link off quirk for Toshiba
UFS_DEVICE_QUIRK_NO_LINK_OFF quirk was enabled for Toshiba devices as
these device may stop resonding back on resume after the link
initialization. But we now have proper workaround in place to allow link
off for these UFS devices hence this patch disables the
UFS_DEVICE_QUIRK_NO_LINK_OFF.

Subhash Jadavani d32b34b8f8 scsi: ufs: send request sense after the link reinitialization
After link reintialization, device would set the UNIT ATTENTION condition
for each logical units. UNIT ATTENTION condition gets automatically
cleared on either receiving explicit REQUEST SENSE command or data commands
to LUs. Although some devices may stop responding to data commands if we do
UFS link hibern8 enter/exit after link reinitialization but before any data
commands. This change works around above mentioned issue by sending
the explicit REQUEST SENSE command to all LUs after the link
reinitialization.

The code is currently riddled with "drop the hardware_lock to avoid a
deadlock" bugs that expose races.  One of those races seems to expose a

Maya Erez 1fd10eccea scsi: ufs: Set DBD setting in mode sense for caching mode page
Host sends MODE_SENSE_10 with caching mode page, to check if the device
supports the cache feature.
UFS standards requires DBD field to be set to 1.

Some card vendors are more strict and check the DBD field, hence
respond with CHECK_CONDITION (Sense key set to ILLEGAL_REQUEST and
ASC set to INVALID FIELD IN CDB).
As a result of the CHECK_CONDITION response, host assumes that the
device doesn't support the cache feature and doesn't send
SYNCHORONIZE_CACHE commands to flush the device cache.
This can result in data corruption in case of sudden power down, when
there is data stored in the device cache.

This patch fixes the DBD field setting in case of caching mode page.

On certain error conditions request abort task itself might fail
when aborting a request. In such case, subsequent request aborts
should skip issuing the abort task as it is expected to fail as well,
and device reset handler will be called next.

Subhash Jadavani 2b83d2f902 scsi: ufs: add error recovery after DL NAC error
Some vendor's UFS device sends back to back NACs for the DL data frames
causing the host controller to raise the DFES error status. Sometimes
such UFS devices send back to back NAC without waiting for new
retransmitted DL frame from the host and in such cases it might be possible
the Host UniPro goes into bad state without raising the DFES error
interrupt. If this happens then all the pending commands would timeout
only after respective SW command (which is generally too large).

This change workarounds such device behaviour like this:
- As soon as SW sees the DL NAC error, it would schedule the error handler
- Error handler would sleep for 50ms to see if there any fatal errors
  raised by UFS controller.
   - If there are fatal errors then SW does normal error recovery.
   - If there are no fatal errors then SW sends the NOP command to device
     to check if link is alive.
       - If NOP command times out, SW does normal error recovery
       - If NOP command succeed, skip the error handling.

If DL NAC error is seen multiple times with some vendor's UFS devices then
enable this quirk to initiate quick error recovery and also silence related
error logs to reduce spamming of kernel logs.

Subhash Jadavani c6029bb662 scsi: ufs: make error handling bit faster
UFS driver's error handler forcefully tries to clear all the pending
requests. For each pending request in the queue, it waits 1 sec for it
to get cleared. If we have multiple requests in the queue then it's
possible that we might end up waiting for those many seconds before
resetting the host. But note that resetting host would any way clear all
the pending requests from the hardware. Hence this change skips the
forceful clear of the pending requests if we are anyway going to
reset the host (for fatal errors).

Subhash Jadavani 5474a9f0c1 scsi: ufs: fix the downdifferntial value for clock scaling
"downdifferntial" parameter of devfreq ondemand governor should
be set to value such that if the load is under upthreshold minus
downdifferential, the governor may consider slowing the frequency down.

As the devfreq ondemand governor is now fixed to handle the
downdifferential correctly, update the downdifferential parameter for
UFS clock scaling (which uses the devfreq ondemand governor).

Lee Susman 923e385795 scsi: ufs: adapt UFS unit-tests for running also on external UFS card
It was found that the UFS unit tests fail when they are run on an external
UFS device. This happens when the external UFS device is suspended while
running the unit-tests, and the un-natural flow of test-iosched does
not resume the UFS driver and device in the correct manner.
We solve this by using the block layer power management API for running
the request queue, as well as explicitly resuming the device before
running the unit-tests.

Subhash Jadavani 27475f8c52 scsi: ufs: add retries for hibern8 enter
If hibern8 enter command fails then UFS link state may be unknown which
may result into timeout of all the commands issued after failure.

This change does 2 things (for pre-defined number of retry counts) after
hibern8 enter failure:
1. Recovers the UFS link to active state
2. If link is recovered to active state, tries to put the UFS link in
   hibern8 enter again until retry count expires.

Hibern8 exit can be called from 3 different context:
	- ufshcd_hibern8_exit_work
	- ufshcd_ungate_work
	- runtime/system resume.

If hibern8 exit fails for some reason then we try to bring the link to
active state by link startup but this recovery mechanism results into
deadlock or errors from first 2 context listed above. This change fixes
the recovery by adding proper error handling mechanism.

Subhash Jadavani 3ef1af1469 scsi: ufs: clear fields UTRD, UPIU req and rsp before new transfers
Some of the data structures (like response UPIU) and/or its elements
(unused fields) should be cleared before sending out the respective
command to UFS device.

This change clears the UPIU response data structure for query commands
and NOP command before sending out the command. We also initialize the
PRDT table length to zero which should take care of commands which doesn't
have any data associated with it. We are also clearing the unused fields in
request UPIU for NOP command.

Subhash Jadavani 5c17c2e786 scsi: ufs: increase fDeviceInit query response timeout
fDeviceInit query response time for some devices is too long that default
query request timeout of 100ms may not be enough. Experiments show that
fDeviceInit response sometimes takes 500ms so to be on safer side this
change sets the timeout to 600ms. Without this change, we might
unnecessarily have to retry fDeviceInit query requests multiple times and
each query request timeout prints one error message.

Subhash Jadavani a983672d59 scsi: ufs: reduce the delay before DME access commands
If UFSHCD_QUIRK_DELAY_BEFORE_DME_CMDS quirk is enabled then we are always
adding 1ms delay before issuing DME commands but after experiments show
that 700us delay is more than enough. As we have atleast 20 different DME
commands issued during link initialization phase (as part of UFS device
detection), this change should help reduce the total UFS device detection
time by atleast 6ms.

Subhash Jadavani 7ae6b4b62a scsi: ufs: stub UFS shutdown handler
Currently it seems possible that UFS shutdown handler may race with some
other UFS transfer requests and it may disable the clocks while normal UFS
requests are in progress. Until we fix this race properly, this change
stubs out the UFS shutdown handler. Stubbing out UFS shutdown handler
shouldn't have any functional side effects as UFS device's cache should
anyway be synced by the SCSI shutdown handler. UFS shutdown handler's
main purpose was to send power down notification to UFS device but it
should work even without this explicit notification.

On Qualcomm platforms, there will be many consumers of the source clock
which also supply ref_clk to UFS Device. So even if generic UFS
driver (ufshcd) vote to turn off the source ref_clk, it's very likely that
device ref_clk is still running. Hence some of the qualcomm chipsets have
separate control bit to gate & ungate the UFS ref_clk to device. This
control bit is part of the TLMM register adddress space so it can't be
simulated at clock control bit which means UFS qcom driver has to manually
control this bit to gate or ungate the device ref_clk. This change adds
support for the same.

Subhash Jadavani 6d1dfe171c scsi: ufs-qcom: disable the UFS PHY reference clock during clock gating
If UFS link is inactive then UFS PHY reference clock can be switched off
to save power. As of now, this clock is turned off only during the suspend
(runtime and system). This change extend support to turn off the PHY
reference clock during clock gating (after pre-defined idle timeout) as
well.

Maya Erez ff440b595e scsi: ufs: add Inline Crypto Engine (ICE) support to UFS
In-order to enhance storage encryption performance,
an Inline Cryptographic Engine is introduced to UFS.
This patch adds in-line encryption capabilities to the UFS
driver.

Maya Erez d4878f6366 scsi: ufs: expose APIs to allow compiling ufs_test as a module
The UFS tests are used for testing the functionality and performance
of the UFS driver. In some of the tests ufs_test uses several scsi
and UFS APIs for getting the disc information and sending UFS
specific commands.
The used APIs should be exposed in order to allow compilation
of ufs_test as a module.

Subhash Jadavani 670eb6e844 scsi: ufs: fix setting init power mode
Immediately after successful UFS link startup, UFS link power mode would
be in PWM-G1, 1-lane, SLOW-AUTO mode. But currently we are doing few
of the DME local/peer attributes access before setting the "hba->pwr_info"
to default power mode. If we are doing link startup as part of error
recovery then old power mode might be set to FAST mode and doing DME peer
access (after link startup but before updating "hba->pwr_info" to default
power mode) unintentionally tries to switch from FAST to FAST_AUTO mode (if
UFSHCD_QUIRK_DME_PEER_ACCESS_AUTO_MODE quirk is enabled).

Above issue is fixed by setting the default power mode immediately after
successful link startup.

Subhash Jadavani ce2239ef96 scsi: ufs: add capability to keep auto bkops always enabled
UFS device requires to perform bkops (back ground operations) periodically
but host can control (via auto-bkops parameter of device) when device can
perform bkops based on its performance requirements. In general, host
would like to enable the device's auto-bkops only when it's not doing any
regular data transfer but sometimes device may not behave properly if host
keeps the auto-bkops disabled. This change adds the capability to let the
device auto-bkops always enabled except suspend.

Gilad Broner 74c2d7b04e scsi: ufs: commit descriptors before setting the doorbell
Add a write memory barrier to make sure descriptors prepared are actually
written to memory before ringing the doorbell. We have also added the
write memory barrier after ringing the doorbell register so that
controller sees the new request immediately.

Subhash Jadavani b026bafcac scsi: ufs: change clock gating timeout to 50ms
Clock gating timeout is generally choosen such a way that it doesn't
impact storage benchmark performance numbers. Although current timeout
value of 150ms seems high hence this change reduces it to 50ms to save
power clock gating timeout. Reducing it to 50ms doesn't affect the
storage performance benchmark numbers.

Subhash Jadavani c0bcee07e9 scsi: ufs: add support for hibern8 on idle
In order to save power we should put the UFS link into hibern8 as soon as
UFS link is idle and power measurement of active usecases (like audio/video
playback/recording) show that putting UFS link in hibern8 @ 10ms of idle
(if not earlier) would save significant power.

Our current available solution is to do hibern8 with clock gating @idle
timeout of 150ms. As clock gating has huge latencies (7ms each in enter and
exit), we cannot bring down the idle timeout to <=10ms without degrading
UFS throughput. Hence this change has added support to enter into hibern8
with another idle timer.

Set the bus vector name used for slow-auto mode to be "MIN".
For each power mode a specific bus vector name should be selected
from UFS DTS node. Slow-auto mode was missing this specification.

Mike Christie fe3ed8722e SCSI: libiscsi: fix potential buffer overrun in __iscsi_conn_send_pdu
commit db9bfd64b14a3a8f1868d2164518fdeab1b26ad1 upstream.

This patches fixes a potential buffer overrun in __iscsi_conn_send_pdu.
This function is used by iscsi drivers and userspace to send iscsi PDUs/
commands. For login commands, we have a set buffer size. For all other
commands we do not support data buffers.

Maya Erez b92a001a48 scsi: ufs: clear UTRD, UPIU req and rsp before new transfers
Clear the UFS data structures before sending new request.

The SCSI command is sent to the device within the UFS UPIU request.
As part of the transfer UPIU preparation, the SCSI command is copied
to the UPIU structure according to the SCSI command size.
As different SCSI commands differ in size from each other, we need
to clear the whole SCSI command field to prevent sending uninitialized
data to the device.

The UPIU response doesn't always include the sense data and can differ
in size.
Hence, the UPIU response should also be cleared before the transfer.
